United Merchant Solutions Pre-App Form
Form Instructions:
The purpose of this form is to gather the information necessary to create the merchant application. Please note social security and banking info is NOT required at this time, unless you would like to provide. Once the application is created and sent back for review and signatures, social security numbers from the signers and a voided check will need to be collected for UMS to submit the application.
To facilitate the application process, please provide the information requested when a field contains an asterisk. Once the form is completed and submitted, the merchant application will be created and sent to the contact email address listed on the form, within one business day.
Any questions or comments can be listed in the message box at the bottom of the form below the Owner info section. When complete, please press submit at the bottom and we will receive the form immediately.
